Welcome! I’m Jasmyne Lyons, a dedicated Family Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) with over 15 years of experience as a psychiatric provider and therapist. Hi, my name is Lisa Squires!
ADHD & Autism Assessments often bring folks to me — but what I truly offer is space to finally feel understood. I work best with people who are overwhelmed, unseen, or just exhausted from trying to do it all alone. Many of my clients haven’t always felt fully accepted — whether due to race, neurodivergence, gender identity, sexuality, or difficult past experiences with care. I especially love supporting kids, teens, and young adults navigating anxiety, identity, and the pressure to "keep it together." I also work with adults across all life stages who are ready to understand themselves in a new way.
As a PMHNP, I offer compassionate, holistic care. I specialize in ADHD & Autism assessments, trauma, anxiety, and depression. My approach goes beyond symptoms to empower insight, healing, and growth. I often work with neurodivergent, LGBTQ+, and BIPOC clients. I also integrate nutrition, mindfulness, and supplements where helpful.

Hi, my name is Jessica Lyons!
Hi, I’m Jessica Lyons, a Licensed Professional Counselor and the Lead Therapist here at Transformation Service PLLC. With nearly 20 years of experience in mental health—including trauma recovery, substance use treatment, and mood disorders—I’ve walked alongside people at every stage of healing. I also serve as the licensed supervisor for our clinical team and lead our practicum training program for future therapists.
My approach is trauma-informed, person-centered, and rooted in real change. I pull from evidence-based practices like CBT, mindfulness, and Acceptance and Commitment Therapy—but most importantly, I show up as a human first. As a military veteran, I bring lived experience into the room and hold space for the complexity, resilience, and strength people carry with them.
I believe therapy is where we begin to rewrite the stories that no longer serve us. Sometimes we survive by pushing through—but healing asks us to pause, reflect, and choose differently. If life feels stuck, heavy, or uncertain, now might be the right time to explore a new way forward. You don’t have to do it alone—and you don’t have to have it all figured out to begin.

I am a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) with over a decade of experience in midwifery and reproductive work, specializing in mental health care for all folks during their transitional, reproductive, and perinatal years. My approach to care is rooted in trauma-informed practices, inclusion, and choice. In my practice, mental health care is both an art and a responsibility - one that requires empathy, creativity, a fierce commitment to autonomy, and the ability to navigate the complex emotional and physical transformations that come with the complexities of life. My background as a midwife has shaped my approach to care, allowing me to integrate a person-centered perspective into my psychiatric work.
I offer an integrative and functional approach to mental health care, blending traditional psychiatric treatments with holistic modalities to promote healing and balance. Additionally, I specialize in working with trauma, burnout, high sensitivity, depression, anxiety, mood disorders, and adult ADHD.
